Full Time
Position Mission:Ensure high quality medical care and services by coordinating, scheduling, communicating, and documenting medical appointments and medical issues for individuals with MR/DD. Responsibilities include, but not limited to ensure medical files are up to date, liaison with other service providers and administering medications when required. Two openings Monday-Friday; 7:30am-4pm and Monday-Friday; 6am-2:30pm. NO Holiday, NO mandatory overtime, NO on-call.
Skills and Abilities:
Must have an LPN license plus 2 years experience in the MR/DD field. Professionalism, confidentiality and excellent communication skills required. Must be able to lift up to 50lbs, and also have a valid NYS Drivers License and reliable transportation.

Documents accurate and timely clinical records, reports, and plans of treatment. Acts as part of an interdisciplinary team.
Skills and Abilities:
Requires a Bachelor’s degree in Nutrition and Food Sciences or Dietetics. Requires CDN. Must have NYS licensure from The Department of Education. Skills and Abilities:
A combination of experience and education equivalent to an Associates Degree and one year of experience; and a valid Driver’s License. Good communication, problem solving, and activity coordination skills. Ability to work schedule-inclusive of one shift each weekend.
